Gonioscopy-Assisted Transluminal Trabeculotomy (GATT)

Indications & patient selection

  • Mild–moderate primary open-angle glaucoma (POAG), juvenile glaucoma, some secondary open-angle glaucomas. (PMC)
  • Good angle anatomy (open anterior chamber angle, limited extensive PAS). (Dove Medical Press)
  • Can be combined with phacoemulsification or done standalone (choose combined if cataract present). (BioMed Central)

Contraindications / caution: heavily scarred or synechial angles (extensive PAS), neovascular glaucoma, or where canal cannot be cannulated. (BioMed Central)

Outcomes & safety (summary)

  • Multiple series and systematic reviews report meaningful IOP reduction and medication reduction after GATT with an acceptable safety profile; hyphema is the most frequent complication and commonly self-resolves. Long-term predictors of success include less PAS and less distal outflow resistance. (PMC)
